Title:  Patient-Centered, Data-Driven Solutions to Polypharmacy in Corrections

Objectives:

  1. Examine how culture change can pave the way for improved quality of care.
  2. Describe the purpose and key features of the WaDOC Polypharmacy Dashboard.
  3. Demonstrate how the dashboard presents medication compliance and adherence data.
  4. Explain how providers can use tools like the dashboard to augment clinical judgement and improve patient care.
  5. Highlight how the effective use of tools such as the dashboard can reduce the prevalence of polypharmacy.

Grand Rounds sessions are free to all attendees. CME credit through the American Academy of Family Physicians is approved. Nursing, PA, and Mental Health CEU credit is also possible through agreements between the AAFP and your professional organizations.

Correctional Medicine Grand Rounds are usually held on the first and third Thursday of each month.
